LAMP Inc and Medicare Local will provide links for mental health patients to receive ongoing support.
With the new program Partners in Recovery Lamp is about connecting high need patients with the right services for.
The Commonwealth funded program was officially launched last Friday with the program to run until at least 2016.
Lamp Inc chief executive officer Lorrae Loud said this program was particularly important for people who lived in rural areas.
“It is an innovative program with the potential to offer more support where often people miss out,” she said.
There will be two support facilitators from Lamp Inc who will travel down to Augusta, Pemberton and Bridgetown to try and meet the needs of the clients.
“Our support facilitators have a great understanding of the services and what is available,” Ms Loud.
The program is focussed on people who experience severe and persistent mental illness, have complex needs that require services from multiple agencies and people who need substantial support and assistance to engage with various services.
